For Patients and their Families – how do we help?

Illness and health difficulties change the everyday life of the entire family.
They bring uncertainty, fatigue, and many difficult decisions.

The "Ostatnia Warta" Foundation was created so that in such moments no one is left alone.

Our mission is to be present where a person needs another person most — in chronic and terminal illness, struggles with disability, the loneliness of seniors, and in the last moments of life.

How we help

Depending on the diagnosed situation and needs, we offer:

the presence of volunteers for the sick and elderly,
keeping watch at the patient's bedside (sitting service),
support in the social rehabilitation and activation of people with disabilities,
organizational, as well as in-kind and financial assistance (e.g., co-financing of medical equipment, rehabilitation).

Our help is exclusively non-medical and social in nature. It always only complements the care provided by doctors and qualified medical personnel, without replacing it.

How to request support?

To enable the Foundation to analyze your needs and provide help quickly, we have launched a simplified online form. Filling it out will only take a few minutes. Formal documents (such as Authorizations) will be signed at the next stage of cooperation.
